We have a confirmed reservation for dinner at Cocina 10 on Thursday, April 18 at 6:45 pm on the outdoor patio. We selected this restaurant based on its clear designation of vegan, vegetarian, and gluten-free options on its menu. 

 

The last panels on Thursday end at 5:40 pm, so this reservation time will give everyone the chance to take an Uber/Lyft from Arizona State University to Cocina 10, which is located in Phoenix (a 22-minute car ride).

KEY INFORMATION FOR THE IUPLR/UIC MELLON FELLOWS AND STAFF

​

Conference Registration

​

  • OSSR purchased your conference registration in advance. Please retain your receipts. 


Travel to and from Phoenix, Arizona and from the Phoenix Airport to Tempe, Arizona 

​

  • OSSR staff purchased your airfare and hotel accommodations in advance.

  • Everyone was responsible for proofing the details of their airfare and hotel accommodations and confirming individually with OSSR staff if any changes needed to be made. 

​

Accommodations

 

  • You are staying at the Moxy Phoenix Tempe/ASU Area 1333 South Rural Road Tempe, Arizona 85281 USA.

  • You each have your own room under your name.

  • IMPORTANT: Do not charge anything to your hotel rooms, including room service, hotel restaurant meals, the restaurant bar, or Moxy Grab N' Go. If you buy anything, you must pay for it with your own credit card when you check out. The Mellon grant will not cover any of those costs. Please follow this rule, otherwise it will create time-intensive problems for Mellon Program and OSSR staff members. 

​

Reimbursements

​

Approved reimbursements for the LSA Conference:

 

  • Ground transportation

  • Baggage fees

  • Meals not covered by Mellon, the conference, or hotel reimbursed at per diem rate. We CANNOT offer per diems for any of the meals that are already covered by the hotel, conference, and/or Mellon. 

 

Ground Transportation

 

  • Modes of transportation include train, bus, or driving your own car (or carpooling).

  • Trips include to and from: your home and airport, airport and hotel, conference/hotel and scheduled dinner for Mellon fellows and staff.

  • Please save ride share receipts that include the payment form (usually the type of card and last four digits).

  • If you drive your own car to the airport, we will reimburse gasoline by mileage but not parking. Submit a printed map of your route with the mileage with your receipts (not gasoline receipts). 

  • You will NOT be reimbursed if you rent a car.

​

Baggage Fees

​

  • Your airline ticket should include a free carry-on baggage.

  • Check-on luggage fees are eligible for reimbursements.

  • Make sure you keep a receipt.

  • If you have one printed out at the airline kiosk, you will submit a digital copy (PDF/photo) of it. 

​

Meals

 

  • Meals are not reimbursed via receipts; no need to save receipts for food or beverages. 

  • Instead, the university uses GSA per diem rates.

  • For Tempe/Phoenix, AZ, the per diem rate is $51.75 for the first and last day (travel days), for full day is $69.00. 

  • Per diem by meals for full days: $16 for breakfast, $17 for lunch, $31 for dinner, $5 for incidentals. 

  • The hotel does not offer a complimentary breakfast, but there is a restaurant in the hotel that serves breakfast at menu prices. Do NOT charge hotel restaurant meals to your room; you must use your personal card.

  • Per diem rates are only applicable to meals not provided by the LSA Conference.

  • Lunch may be provided by the conference (TBD).

  • If you do not eat meals provided to you by the conference (TBD), you still will not receive a per diem. We realize this can be challenging for people with dietary restrictions. The rules for per diems are determined by the state of Illinois.

  • You will indicate this information in your reimbursement request spreadsheet, indicating which meals were covered. We will provide a sample spreadsheet for you to make it easier to complete.

How to Claim Reimbursements
 

  • ​Fill out the reimbursement spreadsheet shared with you.

  • Each of you will have a tab with your name on it. See the guidelines above and the sample provided on the spreadsheet if you are confused and reach out with any questions.

  • All receipts must be submitted electronically as PDFs or PDF scans.  You do not need to send in hard copies.

  • To expedite the process, you should have Deanna Ledezma and Marina Álvarez approve your spreadsheet and receipts before you send your files to OSSR. Any errors will slow the process. 

  • Send PDFs of your spreadsheet and electronic receipts via email to ossr_las@uic.edu. Please cc Deanna Ledezma and Marina Álvarez. 

  •  â€‹You should email these documents as a single PDF within one week of your return home from the conference, no later than April 29, 2024.
  • These, along with per diems, are usually reimbursed within six to eight weeks. Please check back with OSSR if you have not received them within this window and cc Deanna Ledezma and Marina Álvarez. The speed of reimbursements is not controlled by Mellon Program staff.
